They may be printed and filled in and/or signed prior to your office visit to save you time: Payment Options:You can pay for your office visits and any treatments by cash, check, credit or debit card at the time of treatment. We also accept most major dental insurance plans. To discuss your treatment options or insurance coverage with our qualified staff, please call (770) 965-4545 in Braselton and(706) 387-0305 in Jefferson. Dental Insurance:We will bill your insurance company under these guidelines. It is very important that you know our services are rendered and charged to you - not your insurance company. We work 100% for you, not them. Dental insurance plans pay only a portion of your treatment, and we urge you to be fully aware of your dental co-payment policy. We will file your claim if we can confirm your coverage in advance, but all charges for that claim will be payable by you if the insurance company has failed to make payment within 60 days of being properly billed. We are pleased to have the opportunity to provide the highest quality dentistry possible in an open and honest environment based on truth and trust.
